[House] Fever instructions from Fernandez, with shopping list

This is, in particular, in case the shots cause a fever, but also
generically useful.

We can use an axillary check until it gets to 100, then we should
shift to rectal.

101 (100.5, really) is a fever.  For a fever, we should give 30 mg
of acetaminophen
