1. Download and fill out a Media Consent form
1. Descargue y llene el formulario "Media Consent" 

2. Email a scan or picture of the Media Release form to kimballe at sfusd.edu
2. Envie una foto o "scan" del formulario a kimballe at sfusd.edu

3. Share a video or link to video of your student dancing to kimballe at sfusd.edu
3.  Comparta un enlace o un video de su estudiante bailando a kimballe at sfusd.edu

The videos will be compiled and may be shared via District Social Media, Teacher websites, and other SFUSD accounts. No names or sites of students will be listed. 

Compilaremos los videos y los podremos compartir a través de media social del distrito, sitios web de la maestra, y correo electrónico de SFUSD.  No aparecerán nombres de estudiantes ni sitios.
