It's not the hospital that determine c-section or not,
it's the OB, and most OBs will do a c if either baby is
breech. I delivered vaginally at cpmc with a breech
baby B but that's because my OB, Dr Lam is old school
(thy don't teach it in med school any more). That said,
I don't think anyone will allow for a "natural" birth.
You WILL get an epidural and IV you WILL deliver in the
OR with only one family member in the very possible
chance of an emergency C. I guess you could tell them
not to turn on the drugs in your epidural if you really
wan to feel pain throughout. I delivered vaginally at
cpmc and it was awesome. Plus with twins we got a
double room so hubby was thrilled. The cot he got with
#1 sent him over the edge.

I had my twins at CPMC naturally. Baby A took 3 hours,
Baby B (who was breech) took about 2 seconds bc they
pulled him out by the feet. Dr. Jason Dimsdale & Dr.
Donna Wiggins (who are in the same practice as Dr. Lam
mentioned above - Golden Gate OBGYN) did an amazing
job. I have to say, I wanted a C-section because I was
so terrified that something would go wrong and that a
c-section would be faster and safer. Dr. Dimsdale took
his time over the course of the pregnancy to help me
understand why natural would be better in my case, if
possible. One thing to keep in mind is that you will,
of course, go in with a birth plan and hopefully your
providers will stick to it as closely as they
can.However, things happen and at the end of the day
it's best to be at the hospital you believe will
provide the kids with the best care.Does St. Luke's
have a neonatal unit if you were to need one?I can only
speak to CPMC - but we had a great experience. Really
warm and caring nurses, great care by our doctors, our
pediatricians did rounds there and were there a lot,
the lactation consultants were helpful and not pushy.
Private room and pretty quiet. I think some practices
have higher rates with c-sections so that may be the
thing to look into vs. the actual hospital.I can't
recommend Golden Gate OB enough and I am really
grateful to Dr. Dimsdale for helping me get to the
place where I was comfortable with a natural birth.
**One other plus for Dimsdale. I was 39 weeks and
ready to be induced - so he had me come in and he
broke my water rather than giving me pitocin to get
things started. I was able to go really long before
needing pitocin. Don't think I got it until I was
actively pushing and my little one was not making her
way.So, I could be wrong but i think my labor was
shorter bc I was w/o pitocin for awhile.best of luck!

Oy....and yes, by natural birth I mean I did not have a
c-section. But, I did have an epidural. And, you do
have to deliver in the operating room in case of
emergency, unlike a singleton birth.

Hi! Laura Norrell at St. Luke's is laid-back, funny
and was very supportive of my hope to have a natural
birth with our mono-di girls. She is also experienced
w/delivery of twins in several scenarios, to include
breech extraction of Baby B w/pain meds after Baby A
(head down) is out. I've read that in some cases, Baby
B may turn head down after Baby A is out.We induced at
37 weeks, which was the target week for our mono-di
girls. Both were head down. After TONS of prenatal
care that helped to determine that both babies and I
were doing well, they were born naturally in the OR (as
others have shared, you do have to be in the OR for the
delivery in case of an emergency). St. Luke's does
*not* have a NICU or special care nursery, so if you go
into labor before 35 weeks (or whenever your OB
determines), you will of course need to go to a L&D
that has a NICU. You also need to feel comfortable
w/postpartum care at St. Luke's because they do not
have a special care nursery or NICU. Your twin(s)
would have to transfer to another hospital if they need
special medical care. In our case, we roomed in with
our twins the first night and we all went home in less
than 24 hours (by choice). I met w/the L&D
pediatrician to talk to her about neonatal care so she
could answer my questions and share her thoughts (she
was very supportive as well). I was there for
antenatal testing 2x week near the end so it was a good
use of my time!If you do go with St. Luke's, you need
to have a back-up plan if your twins arrive early.
Because St. Luke's is a part of CPMC now, perhaps
continuity of care is possible between the St. Luke's
and California St. campuses. If your twin pregnancy is
relatively low-risk, a natural birth is totally
achievable, especially along with the right education
and support. I gave birth w/my first singleton at
home, so I knew what labor and birth was like w/o pain
medication and spent many hours preparing for the birth
w/our midwives, doula, classes, reading, community
support, etc. My homebirth midwives also cared for me
during my twin pregnancy, labor and delivery. It's
very likely that giving birth for the second (and
third!) time was easier for me because I had given
birth once before. So, if this is your first
pregnancy, your labor is almost certainly going to be
longer compared to subsequent pregnancies.You
absolutely can plan for a natural birth if that is
really what you want. You might end up w/an emergency
c-section after all, but, you might not. None of us
knew with 100% certainty how we would welcome our
children into the world; emergency or scheduled
c-section, vaginal w/or w/o pain meds, one vaginal, one
c-section (hopefully not!). And of course the ultimate
goal is the same for every birth: healthy babies,
healthy mama. Lastly, you will have to find solid
support for a natural birth plan because a lot of
people will think you are CRAZY to do it w/twins :)
Will just prepare you for all of the questions and
comments that you will get after they are born
anyway!Feel free to contact me if you have further

Wanted to say thank you to Carin & SFPOM! I already met
my match - Jennie P - and had a great chat about what
was coming down the pike (these are #2 & #3 for me - so
it's the twins part that's huge.) The mentoring program
is a super helpful idea & so generous of all you busy
twin mama's out there to help us twin mama's to be. So

Thanks for all the helpful comments, suggestions &
ideas - super appreciated!I got a better support belt
to start, have increased Physical Therapy to 2x/week
(it's covered by my health insurance - if only prenatal
massage was too!) plus a crazy idea that I read on
another twin blog -- I borrowed a homebirthing tub from
my doula to use at home whenever I want!Fingers crossed
it all helps. As amazing as this pregnancy has been
(have been better able to appreciate it better the 2nd
time around) and as amazing as the double results will
be -- remembering it's temporary is always a good
reminder...I can do this for 12 more weeks if I have to
- in fact I hope I get too!Thanks again :-)

Oh I failed to mention in previous post that I went
from 8 30 minute pumping sessions a day to 4 10 minute
ones. I pump about 22-24 oz breast milk, and supplement
with 1-2 bottles of formula. Like previous poster said,
if you can invest in the 8 Xs / day (including 3-4
hours at night) your body will get used to that
production schedule and you can cut back the # of
times. Also pump until after you're totally empty to up
the production. And drink mothers milk tea.Good luck

Thanks everyone for the advice and shared stories. We
took the plunge with an AuPair from Thailand. She
arrives next month. In addition, we will have our
oldest daughters past nanny, work 1 day of the week. We
are excited to have the extra hands with our girls and
have some time on the weekends to workout and go out!!I
will keep everyone posted on how it goes!
