Night nurse for sure. Ours was so great, she even
cleaned the kitchen at night, so daytimes went much
more smoothly. If you do go for daytime help, you may
just want to find a housekeeper or mother's helper for
much cheaper. The day doula was really helpful for the
first week, so I could learn and get reassurance that I
wasn't going to kill the babies. Then, about a week
into my daytime care, I found it just stupid that
another person was feeding and spending time with my
child while I schlepped laundry and unloaded the
dishwasher.

I agree, night nurse. We have 9 week old twins and
debated the same question. We ended up with the night
nurse and it's wonderful. I wish we could have her more
than we do 2 nights per week.

I agree with using the night doula. If you search by
name, I used Vikki Dinsmore. She is great. As for a
mother's helper, I found one on Care.com, who is no
longer available. But she was so helpful once a week to
help me get the house cleaned. It was amazing.

I too wanted to work as long as possible before -- but
then the doctors recommended my going on leave earlier
(around 33 weeks, I think? Delivered at 38w5d)...
BUT - it's good to get yourself prepared EARLY -
including if your friends/family want to throw you a
shower. Chances are, you might be tired, and a tad
uncomfortable later in your pregnancy, and you will
enjoy your shower more if it's held earlier. (Plus, if
you then need to go out and fill in the still-missing
things on your list, you'll also have more mobility
earlier on.) We had mine a little late in the game,
and it was absolutely lovely - but TOTALLY exhausting.
You really want to rest as much as you can, towards
the end of your pregnancy, and keep those little ones
in as long as possible!Also, if you get a doctor's note
that you need to start earlier, then I think that just
triggers your disability leave earlier - but doesn't
impact vacation/PTO time for many employers. (I
thought it would benefit me to work as late as possible
- but because I went out earlier with a doctor's note,
that wound up not being a problem. (That stuff is
also good to find out early!) Good luck to you!

My doctor was pretty insistent that I stop working at
33 weeks. I was pretty uncomfortable at that time, and
I had to take BART, which was getting tough. I was
induced at 38 weeks. Those last 5 weeks were L-O-N-G
and there were times I wished I was at work so I had
something to do, but honestly, I was so uncomfortable
and it seemed like I was at UCSF constantly for fetal
monitoring, anyway, so I know I was much better off not
working.

Another enthusiastic vote for St. Luke's from me --
they were awesome for us, knowing that baby B was
likely to remain breech. Minimal drugs (altho I
couldn't get out of the epidural at that time (4+yrs
ago) - but they were very respectful of our wishes, and
we worked together to figure out compromises. I was
REALLY pleased with how everything went -- Dr. Norrell
is GREAT, and the midwives were awesome.(But - like
someone above said - you do need to have a back-up
hospital lined up in case you deliver early. We went
with UCSF, and they had a doc there also willing to do
a breech delivery (that seems to only work if it's baby
B... baby A pretty much HAS to be vertex for any
hospital to even consider not going cesarean
immediately.) It's a good idea to meet the back-up
doc, and tour that facility, just in case. The fewer
surprises when the time comes, the better.Another thing
I'd HIGHLY recommend, if you're interested, is the
Bradley birth prep class... it's a time-commitment,
but probably the BEST possible thing we did to prepare
for becoming parents. It was SUCH a solid,
comprehensive (but fun) preparation for not just birth
- but the post-natal stuff, and even a little beyond
that. Small groups, both partners totally involved. I
can give you the contact info for our instructor, who
we LOVED and are still in touch with! (The main shtick
with Bradley is natural childbirth - how to do what you
can to achieve that, and to know what all the possible
interventions are, so that - if you are confronted with
them while in labor - you already know what it all
means, what makes the most sense to you, what questions
to ask, etc. It was incredibly empowering -- and in
many cases, replaced my fear with actual excitement and
anticipation...) They have a very high success rate.
I can't say enough about it -- but should shut up now!

We transitioned around 4 months, and we started by
swaddling them with their arms out during naps with a
full swaddle at night. They slept much lighter with
just the half swaddle, so we stayed with the full wrap
at night for as long as we could. We then moved to the
armless swaddle at all times, and then moved to sleep
sacks over the course of about a week. Halo makes sleep
sacks with arm flaps that can be used if your LO still
likes a full swaddle, but you can then wrap them under
their arms once they are comfortable with their arms
being free.One thing I didn't predict was how much more
active they were with their hands at night once free.
I thought I had trimmed their nails pretty well, until
the scratches on their face every morning showed me
otherwise.Good luck!

We just finished with the swaddle last week. YEAH! Our
babies are four months old. We used the Halo sack, 2 in
1. We started by unswaddling one arm at a time,
alternating at every nap. After a few days, we
unswaddled both arms during the day. Eventually, we
unswaddled both arms at night...and they did it! We
still use the sleep sacks because this is SF and it's
generally not warm at night.

Happy to be a mentor! We have 6 month old fraternal
girls, live in an apartment in pacific heights. I work
part time variable hrs, so pretty good availability. I
was lucky enough to have a friend who did this for me,
happy to pass along whatever we've learned!I can also
throw in a free twin my brest friend and a set of dr
browns infant bottles for anyone who wants them!
