<p><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">We are unable to process the application for Kai Ladue because your child’s proof of birth and your proof of home address were not uploaded. </span></span></p>
<p><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">Please email a copy of these items to <a href="mailto:enrollinschool@sfusd.edu">enrollinschool@sfusd.edu</a> within the next 10 working days. If the appropriate documents are not provided by the stated deadline, your child's application will not be processed, or their present enrollment may be revoked. </span></span></p>

<li><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">A statement by the local registrar or county recorder certifying the date of birth</span></span></li>
<li><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">A duly attested baptism certificate</span></span></li>
<li><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">A passport</span></span></li>
<li><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">Court Order</span></span></li>
<li><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">Health Office/Vital Statistics record of birth certificate date</span></span></li>
<li><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">When none of the above documents is obtainable, an affidavit of the parent/guardian can be submitted.</span></span></li>

<p><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif"><strong>Acceptable Proofs of Home Address: </strong></span></span></p>
<p><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">Must include the name, date and the most recent address of the parent/guardian.</span></span></p>

<li><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">Utility service contract, statement or payment receipts; from agencies such as PG&E, water, cable, or garbage within 45 days (no cell phone bills or bank statements will be accepted)</span></span></li>
<li><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">Automobile registration or auto insurance policy (declarations page), must be current</span></span></li>
<li><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">Homeowner’s or renter’s insurance policy, must be current </span></span></li>
<li><span style="font-size:14px"><span style="font-family:arial,helvetica,sans-serif">Property tax statement, must be for the current assessment year.</span></span></li>
