Dear Customer,



Thank you for submitting your CleanPowerSF Solar Net Surplus Compensation
Cash Out request form.  *In order to complete the process and receive your
cash out check, you must first submit a completed IRS Form W-9.  Please
submit the completed Form W-9 by September 22, 2023. *



There are two ways to submit your Form W-9:

·         *Electronically*:  Reply to the email from the City and County of
San Francisco Supplier Management office (Supplier.Management at sfgov.org)
requesting your Form W-9. You can find a link to the form here:
https://www.irs.gov/pub/irs-pdf/fw9.pdf. *Please do not reply to this email
with your W-9.*



·         *By Mail*:  Complete Form W-9, and include a print out of this
email and the completed W-9 to:



Office of the Controller, City & County of San Francisco

1 Dr. Carlton B. Goodlett Place, Room 300

San Francisco, CA 94102



If we do not receive your completed Form W-9, your Net Surplus Compensation
will remain as a bill credit that will apply to future CleanPowerSF
charges.  If eligible, you will be able to request your Net Surplus
Compensation balance after next year’s annual true up period or when you
close your CleanPowerSF account, whichever comes first.
