2026-27 SFUSD Enrollment Application is Missing Immunization Records

We are unable to process your child's SFUSD enrollment application for next  
year because we do not have your student's health records. Please email  
your immunization records and Tuberculosis clearance to sflink at sfusd.edu or  
deliver it to 555 Franklin St, Room 100 within the next 10 working days.  
You may also submit your immunization record on ParentVue at  
sfusd.edu/emergencycard.

If the appropriate documents are not provided by the stated deadline, your  
child's application will not be processed, and your child may be excluded  
from their school assignment.

IMPORTANT NOTE: If you've already submitted records and received  
confirmation that your child is compliant, you may disregard this notice.
