Letter of ADHD diagnosis for Kai Ladue

Thanks for sending this over.

Attached below are a few documents. One is a prior written notice which
outlines that the district is in agreement with moving forward with this
evaluation, the next is the assessment plan which outlines the various
areas of assessment that will be completed, and the last is procedural
safeguards which outlines your rights as a parent in this process. In order
for the evaluation to begin you would need to look over the documents and
determine if you consent to the assessment. If so you would check the
appropriate box on the assessment plan and then sign it and return it to
me. If you have any questions please let me know.
